SPORTS PHYSICAL FORMS

Does your child play sports? If so, she or he will need a preparticipation form completed before they hit the field. Many schools require a completed form dated after April 1 to allow participation for the following school year.

Most schools use the New Mexico Activities Association Pre-Participation Evaluation Form, which can be downloaded here (English | Spanish). However, you should confirm with your school so your provider completes the correct form during your visit.
